This notice explains how KAPT handles information submitted through the KAPT Digital Clinic diagnostic gateway. It is a placeholder to be finalised by KOBIS Berhad.

What we collect

We collect the details you provide in the diagnostic form: your name, work email, mobile number, position, organisation details, your description of the challenge, and any files or links you choose to share.

We also record a submission timestamp, a one-way hashed indicator of your network address for security, and your browser type. We do not store your raw IP address.

Why we use it

Information is used only to review your submission, prepare an initial digital diagnosis, and contact you about it. We do not sell your data or use it for unrelated marketing.

Please do not send sensitive data

The public form is not a secure channel. Please do not submit classified, restricted, confidential, personal, medical, financial, or legally protected information. We can arrange a more secure process where required.

Who can see it

Submissions are accessible to authorised KAPT team members for the purpose of review and response. Access is controlled by role-based authentication and recorded in an internal audit log.

Retention

We keep submission records for as long as needed to support the enquiry and any resulting engagement, then review them for deletion in line with our retention practices.

Your choices

You may ask us to correct or delete your submission by contacting us with your reference number. Some records may be retained where required for legitimate business or legal reasons.
